The former Bachelor contestant competed on the 12th season of the ABC show, on which she played to win the heart of Matt Grant.
The 41-year-old was the pilot of the small plane that crashed in Los Angeles.
An eyewitness, Byron Mayes, was working for Best Drilling and Pump, Inc. when the aircraft hit the ground across the street from him.
He said: "All of a sudden, my partner started yelling, 'Watch out, there's a plane coming!' and it looked like the plane veered up, lost control and hit a couple feet away from the truck, a couple feet away from us.
Mayes said he and his colleagues ran to try and help, and pulled Storm, whose legs were on fire, from the aircraft.
He added: "Our first thought was getting her out, making sure she was OK."
The former reality star worked for Pacific Blue Air, a training facility at the airport, and she was wearing her uniform jumpsuit on board, according to NBC News' witnesses.
Firefighters were dispatched at 12:02pm to put out the flames remove the Erin from the wreckage.
She was in full cardiac arrest when emergency services arrived and was transported to an area trauma centre, according to LA County Fire officials, however she did not survive.
Shayne Lamas went on to receive Grant's final rose in 2008, and she has spoken out about Storm.
