The QPR fan was punched in the head after racing over to confront the supporter during the Championship clash at Loftus Road on Saturday.
A left hook was thrown by the Birmingham supporter after he jumped onto the pitch in front of the away end.
The two fans were later dragged away by a steward.
Three people were arrested during the west London side’s 2-0 victory.
Two police officers were injured in the violence with colleagues having to use batons to restore order.
A Hoops fan suffered a broken nose during the match.
A number of supporters also sustained injuries and were treated by police medics.
An investigation has been launched by both clubs into allegations a coin was thrown at QPR players as they celebrated Junior Hoilett’s first-half penalty.
“From around 14:30hrs on Saturday February 27 a large number of Birmingham supporters caused a disturbance in Ellerslie Road near to Loftus Road Stadium,” a Met Police statement read.
“Cordons were put in place to assist police officers in ensuring public safety.
